Hazel Mae Hartley 68, of Amsterdam , Ohio passed away Monday, December 8, 2014 at her home. She was born November 21, 1946 in Moundsville , WV , a daughter of the late Everett and Florence Jessie Hartley.

In addition to her parents she is preceded in death by two brothers Clarence and Albert, a sister, Leona Buckel.

She is survived by three brothers, Everett "Jim" Hartley, Jr. of East Liverpool , OH , Carl (Debbie) Hartley of Ruther Glen, VA, and Bob Hartley of Bloomingdale , OH ; a sister, Jean Newlin of Bloomingdale , OH , several nieces and nephews and great and great-great nieces and nephews.
